14. Recording and Reporting Requirements.
a. Reporting Compliance Suspense Date (Aircraft). Upon entering requirements of this TB on DA
Form 2408-13-1 on all subject MDS aircraft, forward a priority message, datafax or e-mail to Commander,
AMCOM, ATTN: AMSAM-SF-A (SOF Compliance Officer), per AR 95-3. Datafax number is DSN 788-8643
or commercial (205)842-8643. E-mail address is "SAFEADM@REDSTONE.ARMY.MlL". The report will cite
this TB number, date of entry in DA Form 2408-13-1, the aircraft mission design series and serial numbers
of aircraft in numerical order.
b. Task/Inspection Reporting Suspense Date (Aircraft). Upon completion of inspection, if the unit has
a bad circuit card, forward a priority message, datafax, or e-mail to: Commander, LOGSA, ATTN: AMXLS-
AEC (Mark Salverson). Datafax and e-mail addresses are in paragraph 16C. The report will cite this TB
number, date of inspection, result of inspection, aircraft serial number, signal data converter serial number,
and date circuit card shipped to TRACOR. Inspection and reports shall be completed no later than 14 days
after task/inspection suspense date. If no bad circuit cards are found, no report is necessary.
