1. INDEX #: A sequential number followed by the Department of Defense Identification Code (DODIC) (e.g.
001-A071). Each sequence number will be unique and will not be reused. If the items to be clustered do not
have an assigned DODIC, the installation will assign a local DODIC for this purpose.
2. CC: Enter the condition code for the lot cluster.
3. ITEM DESCRIPTION: Enter the nomenclature of the item clustered.
4. LOT NUMBER: Enter the complete lot number of each lot in the cluster (e.g., IOP91F012-024).
5. SAMPLED: Enter an X in this column if the lot has been used as the lot cluster sample; otherwise leave
blank.
6. DATE: Enter the date the lot was used as a lot cluster sample; otherwise leave blank.
